Each person is unique and engages through different means. Each person is unique and engages through different means. Habit Care is a dynamic program that engages patients through patient web and phone programs, social networks, phone and peer coaches, and pro platforms. Each of these components is intelligent, personalized, and simple to engage and impact patients into sustained habit change.

Habit Care works closely with your team to customize these platforms to maximize the impact of the program by targeting your needs and integrating any existing programs in place.

Habit Care offers Interactive Voice Response (IVR) system, which allows sophisticated natural 2-way communication with patients for assessments, tracking, recommendations and reminders.

Habit Care can also provide optional Habit Care coaches to develop one-to-one relationships with each participant. All Habit Care coaches are trained behavioral health specialists with nurse and pharmacist support to answer medical questions.

Habit Care’s simple and engaging Patient Web Program offers powerful knowledge resources and actionable advice to help patients make healthier choices
  • Patients take assessment surveys to receive customized advice
  • Actionable advice and tools are accessed
  • Support is received from coaches and social network

Peer coaches, friends, and family can engage with patients and monitor progress via Habit Care’s Peer Coach Platform

The Habit Care Pro Coach platform provides a time efficient platform to tailor the intervention and make progress at each point of contact.

The social network adds to patient support and the ability to connect with each other to increase patient motivation

Reduced emergency healthcare costs are important to Payers. Habit Care provides valuable reporting and presentations that are designed to maximize internal learnings and improve reimbursement from payers

While Payers are well aware that patient behavior and habits drive healthcare costs,
  • Existing efforts such as disease management have proven largely ineffective because these programs lack a strong behavioral science basis
  • Habit Care’s powerful reporting can help deliver evidence needed to leverage across negotiation
