Advice for making lifestyle changes are often non-actionable and sometimes even condescending. As a result, most people understand, remember, and take action on less than 10% of what they are told by their providers, despite the large role that behavior plays in health.

The Impact of Behavior on Health
Patients need succinct, actionable recommendations and resources from their provider, so that he or she can effectively make lifestyle changes. Once the patient begins to take action towards making lifestyle changes, it is important to help the patient set goals and track their progress.
Habit Care empowers the provider to communicate actionable recommendations to encourage the patient to lead a healthier lifestyle—when coupled with a specific product or device, research has found that providers feel more at ease prescribing these products.
Assessment:
- The Habit Care Assessment and Recommendation Engine uses simple, time-efficient questionnaires to identify participant readiness, self-efficacy, and environmental factors that affect lifestyle choices.
- Optional/additional assessments, with a focus on mental and behavioral health are also offered.
Motivation and Choices:
- New behaviors are often difficult to stick to unless there is an internal motivation to do so.
- The Transtheoretical Model (TTM) is built into Habit Care by assessing a patient’s "stage of change" to provide the appropriate strategy for enhancing the participant’s motivation.
- Patients need to feel confident that they can achieve their goals. Habit Care utilizes the Self-Determination Theory (SDT) to support self-efficacy and proactive choice-making.
Follow through:
- Habit Care provides a plan for move forward. Tools based on Cognitive Behavior Theory make it simple to set and track reminders and goals and manage motivation and progress.
Support and accountability:
- Change is much easier for the patient if there is someone to provide inspiration, support and accountability in their efforts to change, a coach.
- Habit Care’s simple tutorials transform providers, colleagues, friends and family into effective, supportive coaches.
